You will be responsible for planning, coordinating and providing day to day management and administration of Home Care Packages and ensuring the activities of the Home Care Packages operate effectively and efficiently to meet the identified needs of elderly people &amp;amp; their carers.<\/p>\n\n\t\t<\/div>\n\t<\/div>\n<div style=\"background-color:;height: 20px\" class=\"nicdark_section  \"><\/div><div style=\"background-color:;height: 20px\" class=\"nicdark_section  \"><\/div>\n\t<div class=\"wpb_text_column wpb_content_element\" >\n\t\t<div class=\"wpb_wrapper\">\n\t\t\t<h3><strong>Qualifications and Experience<\/strong><\/h3>\n\n\t\t<\/div>\n\t<\/div>\n<div style=\"background-color:;height: 20px\" class=\"nicdark_section  \"><\/div>\n\t<div class=\"wpb_text_column wpb_content_element\" >\n\t\t<div class=\"wpb_wrapper\">\n\t\t\t<p>To be successful in this role, you will have:<\/p>\n<ul>\n<li>Strong knowledge of Home Care Packages program and well-developed skills in providing assessment and person-centred case management to complex clients.
